In addition to the extra damage mods, her projectiles auto-hit for significant damage if her melee attack hits, and I believe she can also inflict vulnerability.

Well I have blind already through my main hand weapon. And, trust me, it always procs.

So, Grace doubles down on this, and even my "nearby enemies have less accuracy" effect. It's quite hard for enemies to hit me, normally. (This map has multiple mods that help enemies out, which I admit I missed when I rolled it. I'd imagine it's still hard for them to hit me, though, TBH.) Although, it didn't seem like Lady Lovefist, or whatever, was missing me at all. :(

Ok I completely missed the Dark Seer.

Actually dying once in three days is not bad, unless you are above lvl 97. By then, you would even stop doing Sirus or tough map mods like this anyway.

I think the reason you rip here is overconfidence.

Yup, and because they do physical with lightning conversion, they can benefit from the physical as extra fire mod that you had in the map.
