Constantly committing suicide

I only have one character, so everything should be there. I had just finished my first Lord's Labyrinth, I backtracked a bit to grab some missed loot, and it happened for the first time. I committed suicide. I'm not sure how, I was in a basic battle and I dropped health super fast. I thought it was monster-specific, but then it happened in the arena. The only real change was that I acquired "trigger word of winter" at the alter. Maybe it's in my skill tree? It's impossible to play right now without it happening. Any help would be awesome.

I should add that I used some health flasks, but hadn't touched my mana flask.

I just went to an area to test. It happened against the Voltaic Seal in the solaris temple. I was using "heavy strike", but it could have been an auto buff.

Berserker - maybe Rage? Which node of the ascendancy did you select? If you unhide your characters we may get a better understanding of what is happening.

Do you get the ingame message that your character has committed suicide, or is the character just dying with a gradual loss of life?

If you picked up Defy Pain as a Berserker, you'll die fast. It's a garbage node.

I do get the message that my character committed suicide. It happens quick. Big drop in life.

Well read the description of Defy Pain and you'll see what's killing you.

You can refund those points, though it costs 5 passive skill refunds per ascendancy node. If you've got enough saved up from quests I suggest you do it.

Alternatively you can pick up some nodes near the center of the tree that reduce reflected physical damage taken by a total of 60%. Then you "only" lose 40% of your entire life bar every time it procs.

I was able to refund it. Should I refund "Physical Damage, life leeched per second" also?
