Unbearable Whispers V Prophecy "with" Twice Enchanted is sometimes Bugged

On a solo run, I searched the treasure room. No Whispering Tome, the Unbearable Whispers V prophecy is now gone, no prefixes cannot be changed on crafting bench (Still undiscovered), and no Inya's key. I had the twice enchanted prophecy and twice blessed mysterious darkshrine. Twice blessed and Twice enchanted both worked. I noted a few players also had this same issue, but this issue was not specified in detail.

When you try to use the prophecy? Before entering the boss room (including the preparation room ) or else?

Both the Prophecies were listed on my challenges "Prophecies" tab prior to the Uber Lab run. No screenshot, just the empty treasure room with me and minions running around looking for the whispering tome that I've done many other temp leagues. The twice enchanted was referenced on screen, but not the Unbearable Whispers IV. It's like the tome prophecy was overwritten/erased, but I got the 3 blessed enchants. Reference number 2,942,294,692.

well, i test it myself i don't have problem for my run (Unbearable Whispers V Prophecy + Twice Enchanted Prophecy + darkshire)
