Killed but no "Resurrect/Respawn" option

Just as I killed the conqueror, I lost all my life (it's ZERO). Also, I'm stuck without the "you are dead" text and also can not move (as I'm dead).
Eventually, I had to exit to character selection and enter again. Miraculously I was alive (all auras are up).
