The Hydra was the last of the four bosses to spawn, and teleported out at the normal health gate. Water Ball totems are continuously spawning, firing, and despawning but the Hydra never came back.
I intentionally killed myself after a minute or two of waiting, and when I reentered the arena the Hydra is back, but just standing in the arena unmoving. It isn't targetable by anything, including AOEs like Righteous Fire or Blade Vortex. The totems are still spawning, firing, and despawning. Maven is still above the arena but is not doing anything. Just had this EXACT same thing happen.
We were duoing this fight and Hydra bugged out. I don't know the moment Hydra bugged out, other person couldn't even see Hydra but I could. Unmoving, un-targetable, immune to damage. Repeating Totems coming up. We did open into a London realm from a DC Hideout. Also noticed ground effects from duo's abilities did not dissipate after Hydra spawned.
