[HELP] - Sometimes Pop Like a Pimple

Over the last couple of days I put together a Frostbolt/Ice Nova Hierophant - https://pastebin.com/s01w4Zjc - based on this video from Esoro https://youtu.be/qEHQ_r_sL6w?t=1

Any tier map or heist I will instantly die (or damn near instantly) if I take burst physical damage. Been bouncing between CWDT Steelskin and Immortal Call (as recommended in the video) but it doesn't seem to help much.

Maybe I'm doing something wrong since I'm sitting on ~4k life, ~1.8k es, ~3.4k mana with MoM and max resistances. The playstyle seems pretty fun but instantly dying ain't.

I don't really want to retire this one so any help is greatly appreciated.
