How to get [+(21-25) to Dexterity] on crafting table?

Like the title says, I'm trying to unlock the level 2 dexterity crafting bench so I can place dex onto my gear.

I've googled around and apparently it has something to do with Helena and/or the map "The Ascent"

I've tried looking around the map, there doesn't seem to be anything I'm supposed to interact with and I'm not sure what I can do to get Helena to show up.

Please help :(

There should be a crafting recipe you can click on at the end of the Ascent level, near the Resonator that transports you to the next act.

I've been up and down this map and all I ever see are the "Resonator Instructions" and the "Lever" and then the "Oriath Portal" once I activate the lever.

I've included a pic of the run through I just did and I didn't clear all of the front part of the map, but you can see it's nowhere to be found towards the end.

It should near the foot of the hill where the portal to act 5 is located

I finally found the crafting recipe and it said I already had it unlocked but when I go to look at my crafting table, I only have 2 dex mods that I can choose from, the level 1 and the level 3 mod.

dude.....hybrid two attributes mod is another mod

you click the level 3 single attribute mod , it will show a drop down list of level 2 and 1 if you unlocked it
