Prefixes / Suffixes cannot be changed

I try to get both recipies. In the description of the crafting bench there is written:
Prefixes: "Unbearable Whispers V Prophecy"
Suffixes: "The Unbreathing Queen V Prophecy"

As everyone knows, this developer team running PoE is not friend of clear and unmistakable words. I solved both prophecies, no crafting recipe appeared in the map after solving the prohecies, nor did a crafting option appear in the crafting bench.

I really don't understand
a) What to do as i absolutely did what the description told me to
b) Where to find sufficient information about this - it's not ingame, where it should belong to, nor is it placed in the game's wiki under 'crafting'.

Could someone please be so kind and tell me what to do as the 'description' by the 'developer' team obviously doesn't contain the answer to this. Always cool to be left behind that curtain of missing information all over the game...but this seems to be it's basic concept. Just die and try to find out why and how.

a clickable item spawns RIGHT when you complete 'part V' of each of those chains.

not at the end of the map, not in the boss room, but RIGHT WHEN the prophecy completion pops up. Next to you.

Both those recipes (and the ones unlocked from the Pale Council) have the shape of a Atlas of the Worlds Shaper Memory (if you came after War of the Atlas, you have never seen them. They are scroll parchment, opened, floating midair that spins)

For the Unbearable Whispers V (prefixes), there will be a recipe in the last Aspirant's Trial, after defeating Izaro and the Whisperer's Tome, which will spawn Inya - which when slew will yield the Key and a chance for the Mind of the Council. You will find this encounter in the room with the Ascendancy Altar and Enchantment Font. You will know it because it will be way more dark and purple rather than white and gold.
Any Labyrinth will work - from Normal to Harvest-empowered.
If you die/logout/portal-out after entering the Aspirant's trial (which means: during the Izaro fight, or during this encounter), you will still consume the Unbearable Whispers V prophecy, and you have to spawn it again. This is different from "Twice Enchanted" prophecy, which will be consumed after you defeat Izaro, rather than after you enter the area

For the Unbreathing Queen prophecy (Suffixes), all you have to do is open a Graveyard map, then find Volkuur and kill her, yielding Volkuur's key and a chance for Grip of the Council.
You will know the area has Volkuur because the area will be more dark and purple than usual. Also, all of the natural monsters inside the map are removed, and changed to Volkuur's skeletal guards, Ancient Heroes and the 4 Bosses you had to kill before reaching Unbreathing Queen V. The Map Boss will still be there, and will yield map completition/awakening as usual
You will consume the prophecy the moment you open the map. This means that if you open another map over it, crash the istance or close all of the 6 portals, you will lose the Prophecy and you have to find again Unbreathing Queen V prophecy.
Only Volkuur has to die - no need to kill any other monsters, boss or Map boss inside the map

Of course, after the recipe comes out, you have to click it. Failing to do so will not be refund.
