Question about Dropping a Headhunter in 3.10

So my brother just got super lucky and dropped his first HH this night from Kitava in a Belfry map.

I was wondering if the "Rare Monsters have a Nemesis Modifier" mod he probably had on the map is enough to drop a HH since you cannot chance one with just that map mod right?

Or is there some Sextant mod or something else that could have made him drop it?

More likely from an incubator, but if not probably delirium unique drop since it doesn't appear to be limited by the standard restriction rules.

If it was specifically from Kitava and was already identified he got lucky from having his awakening bonus.

The awakening bonus gives % chance for the map boss to drop an additional unique item. These additional unique items are not restricted by league specific items.

Can incubators and legion chests also drop headhunter?

"Time-Lost" incubators can drop a HH. I'm not 100% sure on the legion chests but I don't think they can because I've never seen a league specific item from them?
