I'm on to kill sirus quest. Before i start, i want to know what happen if i spend all the portals? Do i have to redo the wathstone guardian thing again?

you have to kill each guardian ONCE (so 4 total kills) to spawn Sirus again.

eh, you have to keep spawning them to get your watchstones anyway

Sirus is easy. You won't have to use all portals.
Just watch the video before fight to learn his mechanics and you'll have no troubles with him. The way campaign should be played by your second and any subsequent characters: https://www.pathofexile.com/forum/view-thread/3772827
