Audio issues

Having trouble with the audio since a few patches but its getting worse and worse
I started noticing irregular audo volume levels at around patch 3.5 but this wasnt bad and i dismissed it as problem for my pc as other programs get muted aswel. this however got worse over time and the music can get realy loud. Since the latest patch however 3.9.2b the loging them just randomly starts blasting trough my headset and wont stop until 5~10 seconds after closing path of exile. this is howver the only way i can make it stop. setting the mastervolume to 0 or any other slider to 0 doesnt help at all and the music just keeps on blasting trough my headset. dx diag of sound devices:
