Missing Forge of the Phoenix map with three Sacrifice Fragment

Got disconnected when entering the map portal seconds after i activated Zana's Map device, after i logged in, there were no portals and no map neither sacrifice fragments in my inventory or stash.

I lost a tier 16 corrupted rare map which for me is costly, but this can be very frustrating to someone who spent tons of currency on a map and suddenly gets disconnected.
