Izaro will drop an additional Unique Item on Death shrine effect not working

Just killed Izaro in Uber Lab with the following Shrine effects active:
You will recieve an additional use of the Divine Font.
Izaro will drop an additional Unique Item on Death.
Izaro will drop an additional Treasure Key on Death.
Players in the Labyrinth will be affected by Impenetrable Shrine.

However upon killing Izaro he did not drop a single unique item. I fulled cleared the Lab, 4 gauntlets, 4 darkshrines, 3 silver doors, and carried both the first and second stage adds into the final fight.

He did drop the extra key, just not the unique.

Please fix this as i'm fairly sure i've had it happen before but have looted to quick to be certain enough to post a bug report about it.

Also please replace the Death's Door boots that the unique definately would've been.
