gem quality recipe : gems with +Q add up to 80% (or more) might not return 2 prisms

I have read this

IIRC it will use enough gems to reach the minimum 40% then start the recipe again.
In your case it will consume 5 of the 9% gems to make the first 40%, the 5% remainder doesn't carry over, leaving you with 4 gems left totaling 36%. | |

It is not a bug.
Individual gems cannot be counted twice. Thus, what happens is;
