Same issue. I also made the mistake of running enough incursions to unlock a temple run, but I went back to my hideout and accidentally opened a new map and never got to run the temple. Now Alva doesn't spawn anywhere. I can't even find her in The Causeway where I have this quest still. I've heard this can be fixed by entering an incursion and leaving via the portal, but now I can't even do that.

I just solved this problem by finding a new Alva in the Causeway and completing her incursion. I died in the incursion the first time, downed the boss but didnt unlock a door. The second time I killed the boss, unlocked the door, and exited via the pedestal. Quest is now complete. Was a pita.
