I just randomly died in HC with no traps or enemies around. Last level of Ascendacy.

Is there something that kills you like toxic air in the level just before the final boss fight to get your Ascendancy? I didn't see my health decrease before insta-death.

I really wanna play but I only want hardcore. If random deaths are a thing I have to bail :-/

If you're sure you didn't kill yourself, and that it was in the room right before Izaro, the only plausible thing i can think of is a nasty unique mob spawned from a prophecy, that killed you offscreen.
