hits keep missing alot,,, help

my physical hits keep missing despite having a 95% hit chance while my lightning cascade seems to have no trouble with hitting enemies, what the hell is going on?

also i think the virulent spider should be added to the list of mobs that can be desecrated

If you are using Lightning strike??
Well lightning strike scale from projectile and is triggering with melee attack, melee portion can basically target ground, so in fact you may hit but your damage is not even registering?

In my recollection if i remember if you miss melee portion the projectile will miss as well, but do not hold me on that tho, just something i think i remember from patch notes. Tho if you target in general direction projectile sure got same accuracy so either you hitting ground or got so low melee damage.

i ain't using lightning strike, i'm using cleave with the rippling thoughts legion sword, plus i'm right in the enemy's face & my attacks will sometimes just refuse to hit

what's your accuracy rating?

when you go to Options -> UI is your Networking Mode on Predictive or Lockstep? if your connection is not great and you are on Predictive mode you may be experiencing "Desync" which is desynchronisation between your computer and the game server. basically the game server shows what is "real" in the game and your computer tries to simulate what is happening at the same time, but if your computer and the server can't talk to each other then sometimes the computer lies to you to make the gameplay experience smoother. so it can look like you are right next to a monster but the actual position of the monster could be in the next room.
