What is this debuff?

I died very fast after i got this debuff. Anybody know what it is? I'm using Zerphis last breath and had all flasks up at the time.

It isn't amoung the status icons in PoE wiki and I can't remember having it seen before.

It is clearly a debuff.
The red icon in the center is usually used for life related stuff.
The blue bubble around oftern refer to energy shield related stuff.
Maybe it is something draining life for ES?

It would help a lot, if you can add some information if it was a specific encounter/map/area and what mobs you were fighting against.

It somehow "ignored" my Zerphis life gain. Map was Shaped Underground River. Death happened in/near breach but there were other mobs too. Also there seems to have been 1 mob affected by tormented spirit:


I have headhunter but I doubt that's the reason since it shouldn't give debuffs. Gear:


If it is about draining life, it must be very strong (my zerphi gain should be around 20k+ life/s).

Those old league mechanics can be very unintuitive. While by now most nemesis mods have been given a good visual feedback on what they do (good job btw) you have to wiki to get the effect of otherworldly, mark of cannibalism or all the Torments (to mobs as well as pcs). I'd argue some of those don't need to be reworked but could be simply removed from the game, since we most times don't know they were there but feel mildly confused when they tip the scale in some situations. Maybe we will see a big cleanup someday to all content that has become rather unimpactful clutter (I'm looking at you Gorebreaker).

May i know how do you trigger Zerphis last breath? It seems that you reserving 90 percent of your mana. Do you toggle your aura on and off to trigger it?

He's running RF with Mind over Matter. Gives him the mana drain he needs
