Decimation Acheivement

Was wondering if someone could help me out in understanding the requirements for this achievement. I just gave it a go and thought I would have satisfied it but it didn't seem to trigger after killing the boss in the area or after then doing a full clear.
I started with a map with a good number of mods on it and I added on warbands and a sextant.  Last bumped on Jul 13, 2018, 8:22:31 PM

If you turn on Advanced Mod Descriptions under Options -> UI you can count up those pesky multiple-lines-per-single-mod on your Map to make sure you have an 8 Mod Map (this is only possible after using a Vaal Orb on a Map), then you can +1 Sextant and +1 Zana Mod and I'm guessing that would work.

Thanks! Didn't realize that was an option and that you could only reach 8 on the map through a Vaal.
