Repeated crash on Conservatory

This is a case of an instance crash. This can happen for various reasons (A problem with the area, a skill combination, etc), so when you're trying to return to it after logging back in, it no longer exists.

Only happened on this map. After all portal were crashed, bought another map from trade board. Got to boss and it happened again (did not log out or close game).
Ran again 2 days later and everything went fine. Just got around to posting it now. Will completely logging out after the first crash before using portals allow you to use the rest of the portals in the future? Xbox Gamertag - Corrison

I did some more digging on this - there's an issue with totems and the boss, so if you can, avoid using those on the boss in the meantime. A fix for this is included in the next patch.
