Hundred foot shadow in Blood altar gets stuck

I used the Hundred foot shadow in a blood altar and it spawned in multiple times and then couldn't leave the pen it was n. the pen opened but never left it.
I don't have screen shot unfortunately as I killed it before I realized what was exactly happening.
