Equiped Stygian Vise belt disappears along with the abyss jewel in it.

Hello! It happened half a hour ago that after my character death I noticed that my belt ( which I found and equipped this morning ) absolutely disappeared alongside with the abyss jewel I placed in it. The belt was quite decent with 2 resistances over 30 - cold and fire, if im correct, and the jewel was providing 6% attack speed and 3-4% cast speed on minions. Im quite new to the game and uncertain what is the whole procedure when reporting bugs. I believe I should post these bug report reference number digits here 1,788,458,004.
