Can't find the act 5 exit

I explore the whole map twice, This is my 2nd character, I remember the exit is right here, but maybe I miss something or just glitch?

You can't enter the Square from there after you killed Avarius/Innocence. You need to go from Chamber of Innocence waypoint.
