Kill Kitava bug?

I killed Kitava at the end of Act 10 but I was killed by kitava at the exact same time. I was given the -60% resist debuff and +100% soul cost debuff but was not given credit for completing the quest. I killed Kitava 2 more times and it still will not credit me for completing the quest. So is my character perma screwed now? Is there anyway to fix this? I was alive at the end of killing kitava the 2 subsequent times just not the first.

You should have been granted the waypoint for the Epilogue. Can you check your waypoint screen?

I was granted access after I reloaded the game. Originally it was sticking me to A10.
