Witch CI Survivability Need Help

hello i need help with my survivability in my Witch'y named Shinov_Wakamane

here are my profile there was only one 67 lvl so ;)

i preffer to play at ES BUT i die at most bosses at one shot ...
all resistances are 75% chaos are 100% but phisical dmg now are insane big for my build what i should change to get better survivability ?
which item will gove me more leech from dmg or spells (is there any?)

mostly i dead on 4 act end boss (Malachai)

3k es is really low, and you dont have to kill merc mal. like at all. farm dried lake for better gear.
