I have not actually gone to the trouble of tracing PoE, but I can tell you that in every single other online game where people have asserted that Nagle is a problem ... a trace shows the developers explicitly turn it off for their connections. Which is not really surprising, because this is hardly a new thing, and developers in the gaming industry...

...well, I guess they would have to be pretty naive not to have heard of it, and not to turn it off. (Leaving aside that most people, these days, don't build the engine, so the engine will just do it for them.)

QoS is ... really not going to be doing what you think it might when enabled, starting with the fact that it is advisory, and on the Internet nothing pays any attention to those markers, even if they are present. It might work if you have a local network run by professionals, and this was a significant feature explicitly requested, but even then ... not gonna do nearly so much as you think.

In general, any "network optimizer" is a placebo: people use it, because they think it will do something, and because they think that they "see" improvements. Even though they were given a lump of sugar, not actual medicine. Belief is a powerful thing.

The one, and only one, exception to this I am aware of is on VPN service that offer access to "their own backbone network", which means that they could - in theory, and I have not tested - possibly deliver a faster or more reliable service than the backbones your ISP uses, despite the VPN adding substantial latency to the whole process.
Has anyone found a solution to this yet? I have suffered from the same problems and haven't found any solution. Seems like all the threads similar to this have died out and haven't had any response from GGG.

I'm afraid this turned out to be an issue with my ISP but they never admitted it. I have in the end switch providers and has resolved my issue :S
