After defeating Dominus, portal to act 4 doesn't appear.

As the title says, after defeating Dominus, the portal to act 4 doesn't appear. Tried re-doing it a few times but the problem remains. Quest text says "Find the exit to Act Four". And nothing happens when speaking to Lady Dialla.

If you're looking for a blue portal like Dialla used, you're looking for the wrong thing. The way out is an exit on the right side of the room.
