Path of Exile Crashes with Blink of Gray Screen gtx 970

Hello,

I've had this problem for awhile now, and I've always thought I had a problem with my GPU (which is probably still the case).

Here is specifically what the problem is:
I'm in game and suddenly my main monitor freezes, my secondary monitor blinks gray and goes back to normal, then I have to close PoE (or I can wait and PoE will put me back into the login screen). Sometimes Nvidia will tell me the GPU crashed.
It can almost occur anywhere (the GPU usually isn't even under heavy load when it crashes) such as when I take a TP back into my hideout (Unearthed, if that matters) or sometimes when I'm in the middle of a map.

I've tried doing a clean install of my drivers. Not sure what else I could try, maybe computer reformat, but hoping there are other ways to fix this!

Computer specs:
i5-4590
EVGA NVIDIA GeForce GTX 970
8 GB RAM
ASRock H97M Pro4 LGA 1150 Intel H97

PoE is on a SSD, not a very good one admittedly. I've tried to reinstall it on the HDD but gray screen still occurs.

PoE Settings:
- Windowed Fullscreen
- No Shadows
- No Antialiasing
- High Texture
- Trilinear Filtering
- DirectX 9

Actually haven't ran a stress test on GPU in a long time. Going to do that now and post results

Just did GPU stress test. Gray screen happened. Doesn't seem to be a problem related to PoE!

To those who might run into this in the future:
I'm going to try to enable NVIDIA's Debug Mode which sets the clock to factory default. I won't be testing it for awhile though
