Strange Graphical Stretching

Only happened near that spot, the farther away from it I got, the less it happened.
After about a screen or two it was gone completely, but at times it completely covered my screen and made it very difficult to see the enemies near me, though I could see health bars through it.

oh, old memories of closed beta coming up ;)

can you do a dxdiag report, export it to text and post it here in spoiler tags? devs likely need the gpu chipset and driver version.

also please make sure your gfx card is not overheating. do you overclock your cpu or gpu?

If you don't have Vsync on, try turning that on, sometime the multi-core option causes this issue aswell.

If you need more information just let me know.

My gpu does tend to get pretty hot while playing, up to around 101C, but on this particular day I believe it was somewhere in between 70 and 80C. The only thing I change is fan speed on my gpu, no overclocking of any kind.

I always have vsync on, game just doesn't feel smooth to me without it.

Your GPU really shouldn't go over 80 at all. (It should only go up to 70 at it's hottest)
After 80 your GPU starts to do crazy shit.

"R9 200 Series is only safe for 85c"
Found that somewhere in the description of the card on newegg.

Think we found your issue! Try upping your GPU fan speed, clean it out, maybe get a new one if you absolutely have to.

It's generally safe up to 100c, but obviously the lifetime of it goes down significantly. Fan speeds go up gradually until it gets around 70C then starts going up to 100% very quickly. I live in southern California so my ambient temperatures are pretty hot all year round, especially without A/C. Without getting some crazy expensive setup this is the best I can do right now.

Screen tearing 90% of the time is GPU related, if you have V-Sync on already, then it has to be because its overheating, or its just dying.
