How do item affixes get chosen?

When you use for example an exalted orb on a rare

Does it work like this:

1. Prefix or Suffix is chosen

2. A specific prefix group or suffix group is chosen based on step 1.

3. The tier in that selected group is chosen


OR is it just just one step?

1. All tiers of all possible suffix/prefix are pooled together and one is chosen?

Names of mods: Cruel, Flaring, of Ease, of Piercing(Tora)

These gives "Corpse Song" name. Basic formula :^)

I mean, what is the process of choosing those mods (assuming you exalt a rare or regal a blue for 1 mod)

Does it
1. choose a prefix or suffix randomly first,

2. Prefix got chosen so then the group +% physical damage get chosen randomly

3. %physical got chosen Then the tier gets chosen randomly


Or is it all possibilities/tiers in 1 big pool then one chosen randomly (based on weight) ??
