Fact: There's a quote attributed to Einstein (maybe some other smart dude) that goes "Insanity is doing the same thing over and over and expecting different results"
Fact: There were players who would repeatedly spawn instances and use loading time (or other methods) to determine if a master mission was available or a entrance to a Vaal side area was present. Fact: This behavior annoyed GGG enough that they changed the spawn rates on popular areas to try to block it. Theory: At the same time, they introduced a stat called "insanity" that measured if an account was engaging in this type of behavior, creating "stacks" if they loaded different instanced of the same area over and over. Conspiracy Theory: This stat was used to modify the behavior of the game to punish the players who would abuse this mechanic. I've long believed characters in games like this are given a hidden "luck" statistic when created. It's why my Caustic Arrow Trapper had three exalteds drop from three separate zones before Act 3 normal, while my lvl 86 MF Quad Totem Arcer had a hard time getting chaos orbs to drop. It's why the fucking rogue always gets his raid drops in MMO's.

I'll help you a bit, "mad emperor" , pretty much any villain in this game also, they are insane. It could have something to do with lore. Merveil, Daresso, Kaom, they all went insane.

This theory might have been tongue-in-cheek, but I wouldn't bet against it. The insanity stacking might very well be the number of open instances you have in the instance manager. The more there, the more stacks of insanity you have.
