Prophecy - Severed Limbs

This prophecy doesn't work, at least on normal version, don't know about other ones. I've tried to do it 3 times and it didn't work at any of these tries.

Haven't played the league so I dont know if oyu need a specific piece equipped.
If you do: Unsocket any gems you have in it. It's a temp work around but you should be able to complete it. I've had the same problem. Ran it three times in Merciless.
