My worst ever is accidently vendoring an 8ex amulet as party of a chaos recipe. Had it in my invetory to craft, and needed chaos, and was also on auto pilot. you can guess what happened.
Sure, the lab can be hard, but it's pretty easy if you're properly geared, and not terrible at the game.
