I'm with the "screenshots or it didn't happen" crowd. The various levels of the pyramid only appear to be stacked, they're actually just separate sections of a single zone. If this bug really happened, it's likely because something corrupted the x,y coordinates of where the oversoul was going to pop up.

"On the contrary, I actually... well, I wouldn't say "believe" but I acknowledge it as possible because I remember another thread (from more than a year ago IIRC) where someone saw and screen-shot Dominus in previous level of his map. And worst change is putting almost all bosses in new version of maps into fucking small areas, where you can't kite well or dodge stuff. What a terrible idiot invented that I want say to him: dude flick you, seriously flick you very much.

I wouldn't even remember what button to push for a screenshot. Even if I did learn it, I wouldn't think to use it in time.
