I think the shrine picks a mod randomly from the item and then you get that effect.

For example:

Mutewind rises - cold resist
Redblade rises - fire resist
Brinerot rises - lighning resist
Kill one head and more will rise to take its place - adds x-y physical damage
Axe and maul - strength
Bow and arrow - dexterity
Wand and focus - intelligence
Icy wind whips around you - adds x-y cold damage
A wind brings a scent of smoke - adds x-y fire damage
A Storm approaches - adds x-y lightning damage
Death doesn't wait - x% attack speed

etc.
