multiple Immortal Calls possible or it has shared cd?
Would it be possible to stick Immortal Call into different equipment parts like 1 in weapon, 1 in boots, 1 in chest, etc.

Immortal Call has its own built-in cooldown of three seconds though, which is far more than what CwDT would give. Not going to be much benefit in running multiple copies nowadays... | |
Does the cooldown start when IC procs or when the buff ends?
What's the exact timing on how this setup would work? Cleave > COMK > IC IC > Inc Dur > CWDT |

You can see the cooldown in the hotkey bar if you hotkey it. Just like any cooldown skill. The cooldown starts when it procs.
For your setup, whenever you kill stuff with cleave it would proc IC, unless IC was still on cooldown. Whenever you took damage from hits, it would increase the damage counter for the cwdt-linked IC gem. When you took enough damage to reach or surpass the threshold, it would cast ICand reset the counter. If IC was still on cooldown then it would not reset the counter, but would wait until you got hit again to try to cast it again. Also remember you will lose all Endurance charges after first Immortal Call cast, so even without cooldown consequent casts are much less efficient.
