CWDT Combo Build

Hi.
I am running a Cyclone Duelist with the following gems on gloves and 2H axe, both 4 linked:

Gloves:
CWDT level 1 + Molten Shell level 7 + Immortal Call level 3

2H axe:
CWDT level 5 + Immortal Call level 6 + Enduring Cry level 7 + Molten Shell level 9

Question:

1. If I am to add Decoy Totem in gloves, what is the highest level it could reach? Can it bypass the level requirement of CWDT or will its level be limited?;

2. When CWDT level 1 triggers after taking 440 physical damage, WHEN will the CWDT level 5 trigger, after additional 260 physical damage or will the damage count reset? Both molten shell skills go to cooldown at the same time even if I only took light damage (I tested it with one skeleton archer, when molten shell triggered, both molten shell from CWDT level 1 and CWDT level 5 went to cooldown at the same time).
1. Decoy Totem is an Active skill gem, so its level will be limited.

According to the wiki - http://pathofexile.gamepedia.com/Decoy_Totem - the highest level Decoy Totem with required level 31 or lower is level 8 (requires lv 28).

2. I don't quite know/remember the answer to this question offhand, so I'm going to use the info on the Wiki to make an educated guess.

http://pathofexile.gamepedia.com/Cast_when_Damage_Taken says that "Each linked spell has its own cooldown and internal damage counter." I presume this means that when the first CWDT goes off, Molten Shell itself has a cooldown of 250 ms (across all gems) and the damage counter never gets to 700 damage, so having Molten Shell and Immortal Shell on two different links would be pointless.
