Elemental equilibrium applies an "EE debuff" on enemies you hit. There are several types of EE debuffs and they all correspond to the elements you hit with. If you hit an enemy with fire damage, they get the EE debuff "+25% fire, -50% cold, -50% lightning". If you hit an enemy with a single spell that does both cold and lighting damage, they get the EE debuff "-50% fire, +25% cold, +25% lighting".
However, each enemy can only have one EE debuff on them at a time. If a new EE debuff is applied, the old one is overwritten. So if you hit with a fireball to give "+25% fire, -50% cold, -50% lightning", then following up with a pure lightning attack will first deal extra damage (due to -50% lightning resist) and then replace the EE debuff with "-50% fire, -50% cold, +25% lightning".
The whole point is that you gain a huge benefit for alternating between different elemental attacks. You can cast fire-cold-fire-cold-fire in succession to take advantage of the lowered resists each time, as long as you never cast the same element type twice in a row.
And... I was wrong about totems' interaction with EE. I found this post by a dev:

Totems/Traps/Mines don't have their own skills - they're linked to you and cast your spells, with your stats, just like you would, so they'll inflict EE if you have it.
So totems apply elemental equilibrium as well. Forgive me for saying otherwise!

sorry for the necro, google searched this topic is the most popular. My question is, so if you hit with a spell/attack that consist of both cold + lightning dmg, it would still be -50% fire and not -100% fire? So what then is the point of hitting with 2 simultaneous types of dmg?

The point in using EE with two elements is simple. If you use a Fire spell the monster would get +25% Fire Resistance, -50% Lightning Resistance, and -50% Cold Resistance, then you would deal cold/lightning (in the same attack) would yield 100% increased damage because of the decreased resistances to both elements. This of course would cause a new EE debuff on the monster with +25% Cold Resistance, +25% Lightning Resistance, and -50% Fire Resistance. So with this, there is an advantage to using all elemental damage but you must alternate like this: Fire Attack/Spell - Combined Cold/Lightning Attack/Spell - Fire Attack/Spell - Combined Cold/Lightning Attack/Spell - Fire Attack/Spell. From a pure burst dps standpoint you should start with a single element (fire in this example) so your 2nd attack with be given a burst with both other elements.
Edit: It does not matter what element you start with. IE you can do cold first then combined lightning/fire, or you can do lightning first (my favorite because shock increases damage), followed by combined cold/fire.

What about Flame Totem? It is a Totem that throws fire but it's not really one of your own spells. So, my question is , would Flame Totem apply Elemental Equilibrium to give you the advantage of -50% Res if you use cold/lightning spells just after him?
