3 pieces needed for the apex were used at the dark alter, boss died, quest not complete though

i think i was bugged/disconnected while fight vaal. then i think he died when i was not around, therefore never 'officially' completing the quest..

i keep joining parties and kill vaal, but i can never exit to act 3!!!!

plese help mods :(

Talk to the bandit or the guy in town that gave you the apex and kill Vaal again.
