First char, Sweep+Leap+HeavyStrike, problems progressing

Hello,
This is my first char in poe, I was looking everywhere for any info that could help me build a proper character to make it to the end game without sweat but things are getting pretty hard after 45-Level.

This is my current build

I am at the 48th level with some relatively good gear. all my resistances are 75 and -20 for chaos, I have 3500 Armour with the aura and I use endurance cry for even more survivability.

As the title says, I am using sweep as main skill, leap for mobs with projectiles(especially the chaos ones) and heavy strike on champion mobs,elites and bosses
My dps is 400 for the main hand and 900 for the heavy strike. I need 4-5 sweeps to kill normal skeletons in vaal ruins.

It is going harder and harder to progress in the game, I definitely need some help on the skill tree

more life, it scales with leech and regen.

For melee go for 200% health as MINIMUM.
