Quest rewards sometimes reverting to first choice

Just a random bug that caused a quest reward to ignore the choice I made, and give me the first (and less useful) support gem. Not very important, but this was the first choice for a good support gem, and it cheats me out of it.
Also, shop items sometimes bug out, not appearing in your inventory until you update something

Also, is there a way that I could swap the gem it gave me for the one I wanted, the piercing support gem? Thanks for anyone who helps!
