Odd Ritual Interaction in a Petal Beach Map

Was doing the final ritual when my character just completely broke and stopped working completely, no movement, attack, animation, or anything. Was completely still in place. Then when interacting with the ritual after portaling back in, the item I clicked on to receive completely changed into a different item when it went into my inventory. It was still a heavy belt, but the prefixes/suffixes all changed to something completely different than what was shown in the ritual window.
