"Failed to connect to instance"

When I move between areas, fairly often I get DC'd to menu with a message that I failed to connect to instance. It happens once every couple maps.
The map instance is fine and I can reenter and keep playing. Sometimes it happens when I try to reenter a map I previously entered without issues.
Is this an issue with the server or my ISP? I want to make sure before I take it to them.
