Herald of Ash fails to apply a debuff that deals damage

Not sure why this is happening, PoB (which just got updated to calculate HoA dot dps correctly) does not indicate I should be hitting any dot cap or overflow. The boss has the visual effect on him. I have no other skill or effect that can hit the worms and cause a low damage HoA dot.
