Vigilant Strike is not applying fortify

After Scourge released, vigilant strike only periodically is applying the fortify buff.
I have vulnerability on hit. I attack with Vigilant Strike, vulnerability gets applied to the target, but most of the time Fortify is not gained. It is especially a problem when attacking a single target but as you can see in the screenshot, this whole pack has vulnerability but I do not have Fortify. It is linked to faster attacks and ancestral call. Probably too late for an answer, but does your character have a "low" amount of damage with Vigilant Strike? Because, since 3.16, Fortify depends on the strength of the hit, meaning that, if you hit for a very low amount, you won't gain any stack of fortification.

Yes! I am playing a level 28 character through act 10 so my damage with vigilant strike is low. That would also explain why it sometimes applies and other times doesn't. I must be close to the damage threshold. I'll 4L it.
