Inscribed Ultimatum fail

I got an inscribed ultimatum for div cards it clearly stated survive!
you place 3 divination abandoned wealth and get supposedly double the amount.

as far as am concerned the ultimatum was successful i survived did not die yet it failed! just lost my 3 cards

Whenever you believe a bug has occurred, you should type in the chat command "/bug," and then with the chat log showing the bug report reference number, take a screenshot - F8 is the native hotkey to save in the default folder, whilst you could also use PrintScrn button. You would then want to upload the screenshot to an image hosting site such as imgur.com and then share the screenshot here.

However, please note that the vast, vast majority of times people have "failed for no reason," the investigation has turned out that the reason for the failure was one of two things:

1. Ruin. The modifiers which can apply Ruin are Ruin I-III, Stalking Ruin I-III, as well as Restless Ground III. You fail any Ultimatum (Survive, Defeat Waves of Monsters, Stand in Stone Circles, Protect the Altar) if you acquire 7 stacks of Ruin, which are generally applied by special monsters hitting you to add 1 stack of Ruin per hit (sometimes 2 stacks of Ruin per hit).

2. Protect the Altar - the Altar dies. This is likely not relevant to you as you mentioned you had a Survive Ultimatum. The Altar's life bar appears over your progress bar and depletes as it takes damage.

Ruin, it is always ruin causing the fails.
