[Xbox] Map is reset when I enter a second time

The above URL shows what happens to my map the second time I enter the same instance. I assume there was a quick fix to that because now when I enter an instance for the second time, my map is reset and I can't see the areas I have already explored. I'm the only one out of my buddies that this happened to, so I was hoping to find out if any other players are having the same issue.

The map overlay resets whenever you leave an area.
Portal to town, Zana mission, Vaal side area, etc.

Still happening two weeks later. Also, when I hit Vaal Detonate Dead while there are Cobra Lash projectiles in flight, the game crashes to Xbox Home screen. I left at 3.9d update and came back to 3.9e completely bricking the game for weeks. Wow. Just wow.

Hi. I'm on PS4 Pro and was having this problem last patch. What fixed it was actually deleting the game from my system and re-installing it. I hope this helps.
