'No Stone Unturned' achievement not working

Hello, as stated in the title after i meticulously clicked on all 140 enviromental lore elements as they are listed in many guides i didn't get the achievement. I double checked and redone everything to no avail.

I listed the completion of all the random ones so i'm sure this is not a mistake on my side. Since my account is pretty old is it possible it's now bugged and counting some old lore elements that are not anymore in the game?
