Legion encounter bugging leech and life flasks while raging?

ever since last patch i noticed a strange bug that i can't replicate by myself, but i noticed it happens mostly with legion mobs with nemesis mods with rage mechanics.
it happens like more or less like this: i am losing life due to the rite of ruin ascendancy while i am raging and then i activate the legion encounter and start hitting stuff and then before the timer runs out i notice that the leech isnt being applied correctly and i have a lot of leech and damage to get full life in less than 1 second.
so when the legion ends and i continue mapping i notice that the life flasks arent working, they just "flash" and stop giving me life but the charges are consumed...the only way to end this bug is porting back to my hideout and reentering the map.
obviously i already died inside the legion fight because of this bug, but since i can't replicate whenever i want i doubt it is something that i can submit with a bug report

Does it look something like this? If so, it seems the health isn't getting synced properly. If that is the case, basically everything is working fine server side, but isn't displayed correctly on your client. That means that your deaths would be because you couldn't see your life dropping, rather than leech and such not working.

In this link, using whirling blades seems to update the life instantly, and the life lost after the whirling blades matches the amount that blood rage would degen perfectly.

it makes sense and could also explains why it seems the life flask isnt working if i was already at full life or close to it if the client isnt synched
but it never happened to me inside delve only with legion encounter but i will test it later again
hope it is not a realm thing since i can only get sub 100 ms ping in são Paulo realm

It doesn't seem related to any specific zone, as another thread had it happen in Caldera outside of a Legion encounter and the video I linked had it happen in Delve.

The video was played on the London realm, so it's not that either.
