Same here. I got her on 0hp but she refuses to die. I suspect that in my case the problem is that a few minutes after entering the the portal to the hideout the game crashed. Another syndicate member has taken the place in the hideout and it was set back to 0% intelligence. However, I found the portals to the former hideout with Tora. Yet she refused to die.

I have the same issue too, I also dont see any mission header to raid the safe house anywhere.. so I think its a bug.
Only thing i can think of is that I had that mission a while ago, but I did not immediately started it and after a while the safe house had to be opened again on the betrayal diagram and then no mission was shown as before. |

I had no problem to kill her, maybe try culling strike or some kind of bug.
